You can find your New Patient Forms located below so you can fill out all your paperwork before you come in to make the process as smooth as possible.
We want to know everything about you, including what you do for work, all about your family, and what you like to do for fun. We also, of course, need to know all about your dental and medical history, any medications you are taking, as well as any fears, concerns, or goals for your oral and overall health.

At Pleasant Valley Dental, we know working dental care into your budget and understanding your insurance benefits can be difficult. That’s why we’re happy to accept all insurance plans and will sit down with you to help you understand your benefits and what is covered.
