Your First Visit
Your first visit is a great time for us to get to know each other! You will be warmly welcomed by our receptionist and walked through the intake process.
You can find our New Patient Form located below so you can print and fill out your paperwork before you come in to make the process as smooth as possible.
We want to know everything about you, including what you do for work, all about your family, and what you like to do for fun. We also, of course, need to know all about your dental and medical history, any medications you are taking, as well as any fears, concerns, or goals for your oral and overall health.
We want you to know you will never be judged at Pleasant Valley Dental. We understand dental problems better than anyone, and our passion is to help the folks who need it the most. We’re here for you!
Insurance & Payment
At Pleasant Valley Dental, we know working dental care into your budget and understanding your insurance benefits can be difficult. That’s why we’re happy to accept all insurance plans and will sit down with you to help you understand your benefits and what is covered. We will file your claim for you to make things easy and help you maximize your hard-earned benefits.
If you don’t have insurance or need additional help working dental care into your budget, we would be happy to speak with you to see if we can’t come up with a plan that works. We seek to remove any obstacles that may prevent you from seeking the care you need, so please let us know if there’s anything we can do to help!
We also accept cash, personal checks, as well as major credit cards.

To cancel or reschedule an appointment, please call us. Please let us know about any change of plans as soon as you can, so we can offer the space in our schedule to another patient.

Yes, we have free on-site parking available for all our patients. We are also easily accessible by public transit.
